      Fees & Required Information

1. Please have your LiveScan Pre-Registration form completed upon your arrival (see PDF file above). 

2. If you are requesting a background check for employment or licensing purposes, you must have the ORI number, AUTHORIZATION number & position applied (Private Party Request only.)

3. You must bring a valid form of Government Identification with a photo such as a driver’s license Passport or State Issued identification.​

 

**Please have a completed LiveScan pre-registration applicant form to include the Authorization Number and ORI Number at the time of your appointment,  or we may be unable to process your fingerprint request**
